A online questionnaire with score

We are creating a questionnaire for HR dept operators who will interview applicants. We start from entering applicants name and setting some additional options on Start page. Then we go step-by-step from first question to the last one. Each question page has a button “Mark as completed”. When it’s selected we consider the step is done. We highlight step ID in process-bar (Step 1 | Step 2 | Step 3 | etc) as completed. The questionnaire can not be submitted until all steps are done. Each step has questions, possible answers, sometimes text inputs and comments field. From each step we collect the following information: 1 – Score – numbers we will summarize to get Output Score 2 – Comment attachment – a text we will clue together with comments from other steps in Output Comment. Each page has a #backend area where you find all the details on how to process on this particular step. In some cases when a particular combination is selected step may overweight entire Status and Output Comment with his own short comment. All these also noted in #backend fields. Script also has a stop button and it calls a stop modal window. All fields here are mandatory. This part ignore if any of steps are not done and it stops the questionnaire with rules described in #backend. Script also has a list of all questionnaire with date range segmentation and search option. I will submit GUI/prototype as Axure file.
