ServiceStack example application with Winform client

Hi there,
The project is to create a basic example application based on the ServiceStack ASP.NET Empty template (from Nuget). The Service/Application host should be written in .net 4.5 using ServiceStack 4.x to be hosted in IIS. Details of fields for POCOs written below. Ormlite to SQL Server should be used.
Winforms Client
The client should be Windows forms using ServiceStack.Client.
There should be 3 forms:
1. Countries
2. Contacts
3. Companies (Main form)
Please add basic unit tests and service validation of data.
