Web app prototype design

For inhouse simple ad publishing system, a simplified cms system, web application we need an initial design in html / css / jquery of the user interface. The basic layout is to include 1. A toolbar / header that goes across the width of the screen, with space for the name of the current logged in user and a tool icon with a drop down menu. Should also include a search box. 2. A sidebar that is fixed and extends the height of the window. The content of the sidebar is similar to the itunes sidebar. With groups of list. Only two levels are needed and the 'groups' or 'sections' do not need to open and close. The bottom part of the sidebar should open and display progress information for tasks. this is similar to the effect in Coda eg. when uploading many files. The entries must also have mock up of count indicators and images for animated rpogress indicators. 3. The main content area. a. Can contain a list of entries. Typically around 30 entries. This should not be a table. Should instead be a 'block' with a large header, 2-3 lines of text, and a last line with small font for displaying 4 properties (author, department, date created) of the entry. or b. A data entry form. Please provide mock up of single line of text with prompt, a rich text area, checkboxes, datefields, section / group headers. The data entry form must also have a publish and save button. This mock up must be finally delivered in html / css / jquery. It must be complete and look polished including hover effects and any other animations. All required images / icons must also be included. Basicly this is an online advertisment editor. The lists in the sidebar will list the drafts, published and expired ads, pluss some filtered views like per department. The design should be simple but modern. Look and feel like Coda, itunes, mac software, but in a web site context. Full support for ie not important: use features of firefox etc like css ronded corners etc.
