Convert MS Word template into a PDF Form

Hi, YOU MUST GIVE ME A SAMPLES OF YOU HAVE BEEN DONE Looking for an admin/word processing person or team who has full access and is highly procficient in Adobe Acrobat PDF. The task is to create two forms. About us: we are a training college and currently would like to lock down two documents by converting them from current in word format into a basic PDF form. the main reason is to prevent unauthorised format changes to the documents and to stop the document from reformatting when trainer type in comments and it exceeds the preset comment box space. The two documents are as follows: 1) question form (10 pages) 2) assessor report (approximately 50 pages) The form is very simple, just has some tick boxes and comment field. for the trainer signoff I like it to be a drop down list of trainers to choose from. I will need to be provided instructions on how to add more trainers to the list at my discretion for later down the track. We need this fairly urgently - turnaround time is 24 hours so only bid for this job if you have immediate capacity and can begin work on it now. I also appreciate if you have Skype so I can talk you through it before you begin. This is a pure word processing/formatting task. I have attached one sample page of each of the document.
