Create a JavaScript form for me

I would need you to create a simple java-script order-selection form for me, which lets the user choose from multiple options. once he finalizes the options, an e-mail will be sent to me with the options and the number will be passed on to a woocommerce link. The orderfile needs to do following things: 1) A user can select from a MultiSelect DropDown following options: A … Industry A A1 … Subindustry 1 from A A2 … Subindustry 2 from A B … Industry B B1 … Subindustry 1 from B B2 … Subindustry 2 from B (here it is important, that he can select several options. If he selects an industry, all related subindustries will be included) 2) The user can select a Region via Multiselect (All Regions, Region 1, Region 2, Region 3). 3) After pushing a button, the maximum number of datasets available will get calculated and presented to the user (for development, use 100 Datasets per Subindustry and per Region). 4) The user can add all of them to cart, by clicking a button (this button will send the e-mail and call the link for the woocommerce product)
