Contest Brief

Starting a Mobile IV Hydration and Oxygen Bar business: HyJuvenate IV Therapy and Oxygen Bar. Need website set up to take client info including name, dob and phone number. Will also need form for medical information to be filled out, which would consist of answering yes or no to questions. Will have consent to treat attached for client to sign. Will also need for client to be able to chose their chosen treatment, schedule appointment and pay through the site. We like this website for a similar business: https://driphydration.com/about-us/ Will be taking picture of our staff and vehicle soon. Will want those added to the site. Once winner has been chosen will share more information to be included on the site.
