Contest Brief

We're looking to redo our membership registration form. We now have two memberships the clients can choose from (platinum & gold) we need the registration form to be updated to reflect the two choices of memberships available. I have added a sample registration form here & as well as our new membership flyer for reference if needed. I have also added the new brand package which includes the new logo, color palette and icons.

The sample registration form is just an outline of what information needs to be on the form. The company logo & name is completely different. The paragraph on on the top should read as follow:

"Congratulations! You have become an Canadian Quality Home Services preferred client. You can expect our highly trained and skilled technicians to become completely familiar with you and your home comfort system. Regular service of your heating and cooling equipment can save you thousands of dollars over the long haul in energy costs, as well as extend the life of your system. Your membership will literally pay for itself year after year."

*Membership information will be different & based of the flyer provided. On this form we need to show the client both options of membership to check off with prices displayed*

*Same Client Name --> Date options from sample form*

*Break down of both memberships need to be displayed similar to the "comfort club membership" display on sample*
