Contest Brief

I need to change the order of some elements of a wordpress plugin.
I am attaching screenshots with a brief explanation of what I want to fix.

Number one
-Change the position of the title, center it.
-Larger size of the "radio button" circle
-In price types, insurance, I need a drop-down button that shows more information to describe each policy (300 characters in each drop-down button)

Number two
-Remove the car name from the table
- Change the order of columns in the table

Number three
-Improve the view of the input label, order to use on the cell phone
