Need help in UI and UX for an App

Contest Brief

We are looking for mockup for a health and fitness app.
Some similar ideas are healthify, cure.fit. It is like a mix of both the apps. It should include following pages:

1. login page
2. personal information collection page (like weight, height, personal info etc)
3. a diet tracker page
(ability to add foods and quantities)
4. a diet recommendation page (A diet plan suggested with quantities)
5. fitness tracker page (Ability to add fitness activities )
6. a fitness plan page (ability to view a activity suggested with video ,description)
7. a calendar view to look through a person's day for diet and fitness activities.
8. An individual's personal profile to view his stats and some of his metrics.
9. a page using which each persons can look up and select their coaches
10. Chat Page (do 1: 1 meeting)
11. And some common things like payment, FAQ etc.

We need both mockup and HTML of those designs
