Patch design and hat mockup (Branding Merchandise)

Contest Brief

I would like to recreate this hat example for my brand.
I require the patch design in eps / and ai or psd file format to send to a merchandiser to print hats.
The content layout is in the sketch attached. The background colour needs to be blue hex#0f75bc and the font etc all white. The icon is an outline of a hard hat with the outline of half a face.
The text is:

TRD MRK
BLUE CHIP ESGR
GREEN CHIP ENV
