Custom Shipping Rule for DotNetNuke RevIndex Shopping cart

I need a Custom Shipping Rule for my DNN website, the shopping cart we are using is RevIndex: [url removed, login to view]
Currently the shipping is not setup, we do have the Shipping Gateway information and account.
The rule should be setup like this:
* All items need to ship in 1 envelope,
* if ALL items in the carts total weight is less than 13 ounces the First Class/Priority/Express are the options
* If ALL items in the cart total weight of all items in the cart is greater than 13 ounces then the users only option would be USPS Express/USPS Priority.
* I also want to charge $.25 on each order within the shipping rule and we want the ability to change this later.
