Create an short 5 second "YouTube" channel Intro / Animation

I want a short intro clip to be added to the start of all videos for my a new kids YouTube channel.
The clip must:
- be created using After Effects 2014 CC
- include a text template for the video title that I can edit each time the clip is used.
- be between 4 and 5 seconds
- be provided in full 1080p
- be suitable for a family audience
- include the channel logo
- include a picture of the host
- include suitable sound effects or simple music clip
I will provide a channel logo which must be included, as well as a static picture of the host.
IMPORTANT: Any images or audio used must not breach any copyright. Proof must be supplied of originality or permission.
