A bit of Programming and/or Shell Scripting

I would like a script that when I log into linux (linux mint to be exact), go to folder where script is located, once it is run, for it to ask me a few specific things:
File Number ( this title should be able to change later to another value if need)
Date
Subject
Entry ( this title should be able to change later to another value if need)
It could all be displayed at once and I just tab through.
If displayed all at once obviously would be cheaper method which I'm fine with. But if able please present price for method with each option showing separately (like a gui but not)
Once saved I would like it to save in format: [url removed, login to view] (w/o me having to specify this) and it should save in folder where script is located automatically.
All aligned justified in times new roman font.
