C# code to access MYSQL server on EC2 through SSH

I need a simple piece of code written in C# that connects to MYSQL through a Putty/Plink tunnel, on success with a valid login/password display a message and on error display the appropriate error message. This is a very simple piece needed for some other code being written but can be written as stand alone. Just throw up a form with boxes for user, login and database, a button for activation and the success or error message.
