CRM Software For Sales Leads-Server Side

I have a doctors office that has phone in sales leads from radio, print and internet advertising. Currently they are using an excel spreadsheet to manage leads. I would like find someone who can advise on open source server sided software that can be installed and have their sales reps access via an internet connection.

Here is the functionality that I need.

1. Able to import current spread sheet in and set up correct fields.

2. Ability to send e-mail and follow up information

3. Put leads into different categories for different types of follow up

4. Ideally has newsletter templates that we could use to create mothly newsletter

5. Auto responder or "Aweber" type capabilties

6. Scheduling module for appointments if possible
