We need to clone the look and content of [url removed, login to view]
Then, we need to build a database and PHP application to do the following:
1. Store information entered by customer into database.
2. Create PDF file using PDFlib on the fly PDF file generation.
3. Send welcome email to customer with custom PDF file attached.
There should be a status that is automatically set to Pending but can be changed to Authorized by admin. When it is changed to Authorized, the customer should receive an email with link to make payment.
There would also be an administrator login that can view and edit entered data, change status, export csv file of database, and resend welcome emails & payment links.
