Convert Joomla 2.5.9 site to 3.4 and apply new template

I need my existing site converted from Joomla 2.5.9 to 3.4 and in the process I'd like a new template applied (which I have already purchased).
I will be redoing the navigation slightly and will provide you with a breakdown of that but the existing pages and URL's must be maintained or have a 301 redirect applied so I do not damage my SEO.
All apps/extensions will need to be migrated over or suitable replacements setup in their stead. The most important being custom form submissions for each page.

I do intend on having a video play in the paralax slider on the home page load.
