Need a "Quote Request" Form Changed and Layout Improvements Need To Be Made

This is the third time we have tried to hire a Freelancer to fix form on website and now we need to put in a request for a qualified developer that can deliver on a promise. The Last Developers said they could use Laravel but obviously didn't have the expertise to get this done.

You must know how to use Laravel, the current website is made in Laravel. You must also know how to use PuTTYgen - PPK files to access website to make changes. This is not a cPanel type interface. Please do not bid on this project unless you are well qualified to accomplish this task. Please read all of our requirements.

We need to make changes and updates to our website form. We hired another freelancer to do the programming for the form but it has a few errors and layout change requirements. . Once the form is updated and optimized the form need to be programmed so once a potential client completes the Lead form it is emailed to four email addresses as BCC (Blind Carbon Copy) The last developers email is still listed on current lead email and they need to be removed from the form email list.

I have attached the form that needs changes and then attached a photoshopped version of how I want the form to look after the corrections are made.

Please do not bid on this project unless you are well qualified to do this job and can deliver on time with 100% professional results. Please no more unqualified develope
