Dynamic html form - mongodb + php

I have the following html form:

And I need the questions to be collected from a mysql, quiz, true or false, numeric or open text. And they are ordered by means of an index and must meet conditions, for example, show question 3 only if both questions have been answered "yes". As the user answers the questions, the next question is shown, not all the questions are shown at the same time.
We want to create a database in mongodb with some questions. And that the web form (bootstrap template) asks according to the conditions that appear in the database. Necessary requirements: Programming using core php (without framework).
The php code separated from the html code and using asynchronous jquery update the questions.
That the questions appear as the user answers the previous question.
Database in Mongodb. The database is not created yet and will be created from scratch.
Once the project is finished, it will be delivered with the uploaded code and the database replicated in codenaywhere.
