Create Log-In and Register for my site

I have log in and register pages (HTML+CSS+JS) and I need someone to convert it to working log in and register pages.
Requirements:
Log-in:
1) 2 fields - Mail and password.
2) Reset my password - the user provide the registered mail and reset password link will send to his mail.. then he can change his password.. and log in
3) Remember me - User details will save in cookies and the user will auto login until he will logout (or delete the history).
4) User Object will save in session&cookies
Register:
1) 2 require fields: Email and password
2) Approve the policy check button and link to read it.
3) The user will Auto login after the register and will redirect to specific page
4) Welcome Email will send to the user, and he will need to active his account from this mail.. a activation link will do it..
