I run a medical billing business. We use this form to send to our patients.

Question 3 (Health Questionnaire) of this form doesn't work correctly. Once a patient picks 'Yes' or 'No' more then 4 times it starts deleting the previous answers.

Patient need to be able to choose either 'Yes OR 'No' for ALL of these symptoms.
