Interactive Web Form

I have a web site located at [[url removed, login to view]][1]. You can visit the page and see the simple form that I have. I would like to create a form that is more interactive than what I have now. The following features should be available:

- product quantity will be selected from drop down with a set price (we only sell one product). The price will automatically be calculated on the set price (price will be entered in code by me).

- shipping will be automatically calculated after entering a rate (rate will be entered in code by me)

- tax will be charged to Florida residents only when the user selects from a yes/no selection (tax rate entered in code by me)

- grand total will be calculated automatically based on selection

- in my current order form I have a billing information section, this will stay the same with the only difference that there will be required fields that must be completed (I will tell you which ones). If the required fields are not entered the order form page will display with the required fields highlighted in red

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
