need slider script added to hosting plans page

i would like to have this script:

added to my shared plans and reseller plans so it can switch the whole plans price and link sort of like:

Only thing really needed for the slider to change is the monthly, yearly, bi-yearly billing cycles really
The slider would also need to match the design, i have the script available but if the preference is to have a slider developed then please advice.
I'm currently finishing this design so there will be more work followed.
