A path based breadcrumb trail for a wordpress site - repost

I need a path based breadcrumb trail for a wordpress site. The attached image does a pretty nice job of explaining things but I have the following requirements:
1. A actual path based breadcrumb trail (NOT wordpress page/post heirachy based).
2. Breadcrumb items show the page title generated by wordpress, not the html title tag (ie: 'Sport' NOT 'acme enterprises :: sport') or URL.
3. Breadcrumb trail shows a maximum of 3 previous items.
4. Previous items are linked to their relevant pages.
5. There is a » separator between items.
6. The list of items is an unordered list ()
7. There is a static 'Home' button at the start of the list of items.
8. If the user clicks away from the site and then comes back it does not list external sites in the breadcrumb (could be an issue if the solution uses javascript and browser history).
A demo site can be provided, or you can use your own test server.
