211954 Subscribe Email To Many Lists

I have a newsletter optin form on my website but I want it changed so that when a user fills out the form (name and email) and then presses the "submit" button they get taken to a thank you page that will ask them if they want to receive more information on other topics. Each topic will have a checkbox next to each one and if the user checks the boxes, they will be subscribed to those autoresponders as well without entering their name and email address again.

Here is DEMO code of my HTML optin form. The only thing that changes for each of my autoresponders are these codes > "merchantID" value and the "defaultar" value.

I have included a attachment of what I want. The 1st page is what they will see when they come to my website. They 2nd page is what they will see after they signup for my newsletter.
