Wordpress widget needed

I need a quiz widget ready to be inserted in any wordpress blog. I will provide 4 or 5 questions to the developer, with the correct answers, and he/she will build the widget. According to the final score, a message will be shown with a link to a site. It should be designed to add to left/right columns.
I will have to approve design and functionality before payment.
