Need a simple piece of html / php code for aweber

It should be a very easy task.
Basically I need a code for:
A signup form for aweber which includes 3 fields
email, name, company name , which a user can submit.
You can get this straight from aweber (they have free trial), no coding needed here.
The thing I need done is:
The form needs to redirect to an url containing this:
[url removed, login to view]{company name goes here}
basically whatever the user puts in the company name gets added at the end of url string.
and on clicking the submit button the user is sent to that url.
