oscommerce login from separate page

I would like to have the login to my osCMax v 2.0 RC3 webshop ([login to view URL]) from a separate page that contains some information about the business. The webshop will be a make-to-order site where companies can register and order for production to be finalized.

The "retail login" at this site is a solution similar to what I would like to accomplish: [login to view URL]

So far, I have installed the contributions (attached) "require login" and "restricted_website with authorization word". The "require login" contrib is a simple solution that "locks" the site so that the login-box blocks the product catalogue for anyone that is not logged in. The more advanced version of this contrib "login to view the site ER" seems promising, but I have not been able to make it work.

I would like you to create an index.html-page that easily can be modified and with a login box to the webshop. I plan to add pictures and text to the "front page" later. What is important at this stage is that the technical solution works together with the existing webshop.
