PHP, Database and MySQL very simple work for expert

Looking for Expert in PHP, Database and MySQL to implement few simple options.
1) I would like progress bar implemented on my form (picture attached)
2) I have basic admin panel and database also working on my site but it's VERY basic (also picture attached)
a) in admin I like to have more options
field to change admin password
field to send email in case I forget password
(both are there but not implemented)
Settings right in admin panel where I can press button and I can:
Delete user
View user
Edit user
Backup database
Restore database
Export database in .cvs
Import database
Download database to my PC
Upload database from my PC
I know I can do it via phpMyadmin but will is more convenient.

I know this is very small assignment but we will have more work for the right person. Preferably from Europe, that speak English and has working Skype with sound or Team Viewer to communicate faster.
