I am seeking an expert Shopify developer for a simple project where I am looking to add an image upload and text box to the product page of a single-page Shopify site. The product image DOES NOT need to update with the image

Buyers should upload an image and enter text prior to checking out. This image and text will them be accessible in full quality on the backend by Shopify administrators.

The feature should function exactly like the product page checkout found here: [login to view URL] Please see the attached image for what I am trying to create. The style should match the existing template.
