I’m a photographer and have a Gravity Forms Booking Page on WordPress. I would like to make a more professional And polished look to my booking page that using icons along with images to provide a more attractive method of collecting information and at the same time offering service options with up-sells. I would like to provide the client a running total and provide an option to pay a $50 deposit, pay in full, or a custom amount using a credit card (Stripe).

I would also like a custom option using Gravity Forms that can calculate distance from a default address to the customers (end users) inputted address (home to photograph). If the distance is less than 10 miles there is no additional charge. If the distance is more than 10 miles it will add $2 per additional mile to the total price.
