I have an html form php file on my server (built in laravel). I would like a passcode (no username) added to the form that captures the user connected to the passcode when the form is submitted. I would like the data sent to Google Sheets when a user submits the form. The Google sheet is complex. I would like the form data sent to 1 of 13 sheets. It should search for the sheet name, date, and user (from the html form) to find the correct sheet and row to insert the data from 5 fields.
