I would like to create a web page to which I can send job applicants responding to my CraigsList ad. They would be required to answer many (?30) questions via selecting from a drop down list and/or filling in text.

These data would be stored according to the applicants name and identifying characteristics (phone number or email address) on a data base that I could peruse on an excel or similar type venue

I do not currently have any web access and would need instructions on hosting, retrieval and making changes to this webpage
