PHP Information storage script (User/Admin logins) + Basic script design

The below is a cut and paste of my script idea, it's in basic form at the moment but should give you an idea of what I require, not much will be changed. Although this isn't an urgent job, I would like to have the system up and running within the next 1-2 weeks.
