Change Ecommerce Cart Page

I have to add some fields to the cart page of an Ecommerce shopping cart. Simply add the fields, test that they will work and you're done. Here is a link to the page. [login to view URL] The changes are under the heading Personalization. Simply add 8 or 9 fields and a row of check boxes. I need to keep this project under $30, so please don't bid over that amount or we will both be waisting our time. Thank You, Doug
