Create interactive pdf from a word document

Very simple. Just re create the attached word document as an interactive pdf file. What do I mean by interactive? The new pdf form should allow user to fill in all the answers by computer. In other words, check boxes, names, address and any other field that needs to be filled out, should be filled in by computer. I give you an example: in the form under dental history, you see many selection to be checked by the user (bad teeth is one of the options). User should be able to put an "X" to the left side to select the item. This is fairly clear. If you don't understand the request, please don't bid. YOU SHOULD DO THIS BY A PDF CREATOR SOFTWARE. the result file should be usable in adobe pdf readers of any version. I will check this with version 5 and above. I need this back fast.
