Bring PDF forms into Excel and create a form-fill input screen

Hi, Experienced Excel Macro Developer. I have read your instructions carefully. I can develop an Excel file to import pdf file as a background of sheet and arrange it according to cell's coordinates. But as I saw your attached pdfs. I think it will be better if you design Excel Form. I can design user friendly User Form with Command Button, Drop Down, Radio Buttons, List Boxes etc. in excel VBA and populate all fields automatically with Macro. So, I can do this job nicely.
