it's a project for summer swim class. requirements are as below. We will need a page for information such as name, date of birth, gender, mailing address, email, parent/guardian name & phone number, emergency contact name & phone number. Also a place to indicate if they what to be mailed or emailed a reminder for next years swim lessons. Also a place for them to sign waivers. I would like a page that lets them know which levels they have completed. Our levels are P/C 1, P/C 2, PS 1, PS 2, PS 3, Level 1, Level 2, Level 3, Level 4, Level 5, Level 6 - Diving, Level 6 - Lifeguarding, Level 6 - Racing Part 1, Level 6 - Racing Part 2, Beginner Teen, and Adult. It needs to be where we mark they have completed a level to be able to sign up for the next level. It will also need to be age specific. They must be 1 - 2 years old to participate in P/C classes. They must be 3-5 years old to participate in PS classes. They must be 6 years old to be in Level 1 classes but no more than 9 years old. They need to be 10-16 years old to participate in Beginner Teen Classes and 17+ to be in Adult classes. I would also like an achievement page so if they are working on a level, they can see what skills they have successfully completed and what skills they still need to complete to move to the next level. Then we will need the page to register and pay. For example, they may choose the 9:00 Level 1 class for June 1-10. They can choose another level 1 class for June 11-20 but they cannot choose a different level. The class needs to have maximums say 6 people per class and a place to notice participants if an opening occurs. Participants cannot register for more than one class in a date range. Sometimes a parent has more than one child so they would need to be able to register both of them before paying for the class. The class spot is not reserved until they pay for the class. They can transfer to another but it must be done 3 days before class begins unless I do it manually. Once they reserve the spot, an email is sent with a confirmation along with our class information that we hand out. I would like it to be able to hold participants for two years then I can pull the data, store it, and delete it from the program so it does not slow it down. This is if a participant does not take a class within two years.
