Data Entry Window Form - Java

There are 38 columns in a Table. I wanted a Data Entry form which when opened will show details to update the data. There are 4 dropdown fields, some are text/numeric/DateTime and Time fields. Proper validation is needed before sending database request. Must be working on mac and Window. Need full code. Please ask me the database Table. Form when opened will be in center of the screen.
