Advanced registration form

I need a user registration form with various fields (which I will specify to the winning bidder) that will export the data to a MySQL database. This form should use advanced CAPTCHA verification. Instruction on implementation of the form and script may be needed. This form also needs to be able to export certain fields (email and password) to an htpasswd file in the format: --line break-- email:encryptedpassword: --line break-- 2ndemail:2ndencryptedpassword: etc... Finally, the email address provided MUST be BOTH limited to a *.edu ending, AND VERIFIED by sending a verification email to the account entered, and only allowing completion of registration through that form. (These restrictions are similar to Facebook's initial registration system.) I belive this to be a fairly simple project (I just don't know how to do it), and thus expect bids to be closer to the $100 level as opposed to $300. Payment will be sent once project is successfully implemented on my site. Thank you.
