data collection method and presentation

Hi, Basicly i need a daily entry page on our intranet where set people can type a percentage into about 15 items. that data should be stored into a sql database. ? After which i would like to present the daily data into a online(intranet) report where a weekly report is shown with some simple red green or amber depending on the percentage, and the ability do drill down the items, and eventually get to daily entrys. cheers,?
