Complete Paper Forms Using Web Based Forms Software

We’re looking to have some web based software built to help those who find it difficult to write or prefer to complete forms using their computer then writing by hand. Therefore what we’re looking to do is scan in documents, turn them into JPG format (can be another format if better) which turns these documents into almost a background image so we can then overlay drag and drop fields over the areas that would have been completed with a pen but instead assign them with data fields (referenced by tags) so that when someone completes their details online it knows exactly where to put this keyed in data on the paper document. We need the ability to stretch fields and make them the height and width that we require (see the attached image for an idea of what we mean). Also with fields where it’s a multiple choice option we require a “variable” field which can inserted as v1, v2,v3 etc. In the admin panel we’d need this multiple choice option to replace variable with i.e. a tick, a cross, a N/A etc so that it knows what to put in it’s place (we select what to show in it’s place by a multiple choice dropdown/radio button). We’d also like to be able to add a line of code to the website (an iframe) next to the form so that the customer can see in real time that the text they are entering is populating the correct part of the form. Once they click submit there needs to be a print friendly version of the form and also an auto generated PDF and Word version of it.
