Create a facebook quiz application

Hi, I need someone to create a facebook quiz application very similar to this: [login to view URL] You have to copy the design of that. Instead of using flash, you can use other programming code. The app will almost be exactly as above, we'll just have some minor change. The features of the app would be: (see attached for illustration) 1. Post a random photo from 9 photo results 2. Include the user's name and display pic within the photo 3. Text/Description of the photo saying "Take the Which Planet are you quiz" I will provide the needed background and photo results.
