Checkout modification, form, chron job for magento

I need a magento checkout method that forces checked out customers to fill out a form of their added personal information upon purchase but after payment is confirmed. I will provide the required form fields. All added fields need to be stored in DB and visible in customer info in admin panel. After form is completed then they need to see a confirmation page. A confirmation email is then sent to the customer confirming purchase. All copy will be provided. I think this confirmation form. Is a standard feature of magento. Then 12 to 24 hours later a membership email needs to be sent (chron job access is available) which has a script of words and four inputs from the new post checkout, information form that they completed. I need all the customer info stored in the DB and visible on the customer section of the admin panel. This particular site admin panel is connected to another admin panel which I use to manage all of my magento websites. Please only bid on this if you are familiar with magento.
