Create a Javascript Form

I want to have written a basic javascript form. This is what I want: I want the enduser/surfer on a website of mine to be able to click a button, have a form appear with text already pre-populated in it. The form will need to have just basic fields, i.e., friend's name, friend's email, your name, your email and be able then to send that message when the enduser/surfer To see exactly what I have in mind, it's just like a form found at this Web page: [login to view URL] If you look down the page, you'll see a big purple button that says, "Click here to share this video with a friend." If you click on the button, you'll see a form appear like the one I wrote about above. If you enter the information, and click on the "Tell my friend & take me back to the video", it sends that info and autogenerated text to the email address in the email address field. Then, if you click on that "tell my friend" button or on the big purple button, it scrolls up the auto-populated form. Thanks, GR
