Create php forms with admin page - with code generator

I want to create a type of simple MLM website. I will list the basic steps of how the website should work. Page 1 - Create email form Page 2- Create new user signup form( name ,address, city,etc,lobby prize) page 3- create blank page ( I wll provide the code for payment here ) page 4 - User login page ( show status of offers completed, Place to put input a code (10 digit) and when submitted will show paid. ) Show referrals and if they have completed an offer or not ,and if not be able to click and add payment code or paypal button( i will provide the code). create referral links. page 5 - admin page ( show all users and status of completed codes ,be able to manual change status of user to qualify for prize), be able to add how many referrals are needed for each offer. generate all the codes needed and be able to activate or deactivate . only allow one code to be used no duplicates.
