ArcGIS toolbar in VB.Net 2005

I wish a toolbar with 3 buttons. Each button brings up a different tabbed form. I select a feature in ArcView 9.1. Each feature falls into 1 of 3 "categories" - Water, Sewer, Natural Gas. Once the feature (e.g. meter) is selected I want to push the appropriate button (Water) and display a tabbed form containing various attributes of that particular feature (Street Address, Inspection date, etc.). The program should indicate an incorrect button/tab/feature relationship (a manhole should/would not have a Smoke Test Date on the Naturall Gas form).
