Need a complete invoicing/time reporting system written in Access for a Physical Therapy business. System to track employees, clients, and billing.
* **Required forms:**
* Menu form(s) to access all other forms and reports.
* Clients: Personal Info, Insurance Info, Doctor/Case Worker Info, Ongoing Dated Comments)
* Therapist: Personal Info, Payrates, Discipline (3 types of therapists)
* Insurance Companies: Billing details (Address and Phone numbers)
* Time Reporting: Report time with each client to be used for invoicing and payroll reporting.
* Authorizations: All work must have a corresponding authorization which specifies unit of work(billable hours) for client. Time reporting will be tied to this to ensure only work is reported for that has been authorised. Each type of therapy has an associated code. Pay rate (by state) is determined on the authorization and used to bill the state.
* Forms to access and update all reference tables.

* Insurance Invoice: Standard insurance form
* State Billing: Complete billing for all clients laid out like a spreadsheet. Shows insurance pending. (When insurance company does not pay or pay full amount, state then gets billed.)
* Private Billing: When not covered by state or insurance.
* Payroll report: Based on reported time. Also a commission to Therapists who have Assistant Therapists working with them. Bonus for Therapists who put in a minimum number of hours during payroll period.
All supporting tables to be built.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Non-disclosure agreement. 3) Complete ownership and distribution copyrights to all work purchased. 4) Weekly updates required with sample of completed work. 5) System to be completed by end of May.
