Create New Site + Hook into API + Make Changes to Existing Site

I am looking for someone who I can employ for consistent long term work, working on a complex PHP web application with 200 customers. The work involves a small amount of front end development, a lot of back end integration with 3rd party software, creating new features for our application. This job would be the first task. This first job is: 1. Create new front end site, hooking into our iframe form. Our sites send leads (form data) via an API (our own API) to our application. The user submits a form on this site, then it gets sent to our DB and application via an API. I will provide you with a layout. 2. This site needs to be hooked into our API. 3. We need someone to change some features on an existing site. Currently the site has fields see screenshot: [login to view URL] We need to modify the form slightly so we can add a few more fields (1 extra phone number field, change one field to a drop down choice). The form currently sends the data to our API, it will need to be modified so it sends the new data.
