Desktop Application for One-Way FTP

I need a desktop application written for uploading videos to my server. I want my clients to be able to download this application and then use it with almost no technical knowledge at all. Launch the application. Fill out a form with information about the upload (no server info etc.). Browse for the file (or drag and drop it). Click a button. See a message about success or failure. I can provide details via pmb. Eventually, I need this for Windows and Mac. If you can provide both, let me know. Thanks
